Abstract
The rising suicide rate in Iran and the relative failure of existing preventive programs indicate that a predominant focus on the medical-psychiatric approach, without sufficient attention to socio-cultural mechanisms, limits the capacity for the sustainable reduction of this phenomenon. Suicide is a complex and multifactorial behavior shaped by the interaction of macro-social factors (inequality, discrimination, unemployment, deprivation, low social capital), local and ethnic factors, and familial and communicative variables, with certain ethnic provinces in western Iran exhibiting a distinct pattern, including a higher prevalence of suicide among women. Weak coordination among intervention institutions, a lack of rigorous evaluation regarding policy effectiveness, and the impacts of the COVID-19 pandemic on intensifying social vulnerability are considered key challenges. Policy proposals emphasize designing multi-sectoral interventions grounded in health, education, employment, social support, and social justice; active participation of social science experts in the national suicide prevention program; strengthening neighborhood-based approaches to alter cultures that generate violence; teaching life skills, particularly for women and families; managing the media representation of suicide and reducing access to pro-suicide content; combating stigmatization in traditional communities; and developing a sustainable post-suicide rehabilitation system with the participation of NGOs and families. Furthermore, organized judicial and legal interventions to address domestic violence and women-related cultural issues are introduced as a prerequisite for reducing suicide in high-risk hubs.
