Abstract
The continuum of elderly care in Iran faces two simultaneous challenges: on the one hand, institutional care for older adults still lacks favorable social value and can induce familial guilt, while on the other hand, the limitations of apartment living and the severity of certain motor or cognitive disabilities render home care difficult and exhausting. Consequently, caregivers of older adults with severe disabilities experience high psychological stress and are recognized as "secondary victims"; hence, professional training and psychological support are essential to prevent burnout. The mental health of older adults, particularly regarding depression, anxiety, and cognitive decline, is also of critical importance, as depressive symptoms can be mistaken for the natural signs of aging, increasing physical consequences and mortality risks if not diagnosed in a timely manner. The prevention of dementia can likewise be enhanced through lifestyle modifications, healthy nutrition, physical activity, and the control of metabolic risk factors. The development of specialized centers for the prevention, treatment, and care of elderly individuals with cognitive and psychological disorders, the design of care models compatible with Iranian culture, and the establishment of integrated services across welfare and medical domains are key strategic recommendations proposed to enhance the quality of life for older adults and their families.
